On this financial policy page, patients will find important information regarding consultation fees, cancellation and rescheduling policies, surgery estimates, payment responsibilities, and additional medical requirements.

Consultation and Appointments

Consultations with Dr. Baumholtz are $150 (in-person or video). This non-refundable consult fee can be applied to your surgery if booked within 2 weeks of your consultation visit.
Surgery estimates are valid for 30 days.

Cancellations & No-Show Visits

If you need to cancel or reschedule, please notify us at least 48 hours in advance.
No-Show Surgery Consult Fee $150
No-Show Other Appointments Fee $50

Surgery Payments

A 25% non-refundable deposit is required to book surgery.
Full payment is due 3 weeks before surgery (usually at your pre-op visit).
We accept cash, credit/debit card, bank cashier’s check.
3rd party patient finance companies are options. Your relationship is direct with them.

Surgery Rescheduling Fees

More than 3 weeks before or cancellation for medical reasons (including pregnancy): $500 fee
Under 3 weeks before or no-show for pre-op visit: Lose deposit, new deposit required.
Under 14 days before: Lose deposit plus additional $2500 fee.
7 days before, no-show or non-disclosure of medical information: Forfeit 100% of all fees paid.

Refunds & Results

Once a service is provided or a product sold, there are NO refunds.
No warranty or guarantee is provided for results of surgery or non-surgical treatments.
Not seeing the improvement you expected does not qualify for a refund.
Additional treatments or revisions may require additional fees.

Discounts & Packages

Any special offers must be used within 6 months.
Specials and promotions are non-refundable and non-transferable.
If a procedure is rescheduled, special pricing may no longer apply.

Pre-Surgery Rules & Medical Requirements

Medical testing (labs, EKG, clearance) may be required. If not completed, your surgery may be delayed and could incur additional fees.
Nicotine must be avoided 6 weeks before & after surgery. Failure to follow this rule will result in cancellation with no refund.
COVID-19 testing may be required by facilities, even if vaccinated.

Additional Costs & Responsibilities

Medical clearance, labs, medications, and pathology are included in the estimate.
Rescheduling or canceling may lead to additional fees from facility / anesthesia providers.
If a tissue sample is sent for testing, you are responsible for costs not covered by insurance.

Credit Card On File

A valid credit card must be kept on file.
Your card is stored securely and only used for approved charges.
